This printable and reproducible ballot is ideal for use in classes, youth groups, or other types of group participation.

Voting for the next Dirty Dozen & Psalm 151 CD is as easy as 1, 2, 3!

Click here to view the ballot on your computer. Select FILE->PRINT from your menu to print the ballot. Photocopy the ballot as needed. Complete the ballot and mail it to Cornerstone Media (address provided on the ballot).

Important notes:

• Ballots will be accepted between now and December 1, 2007.

• If you would like to order advance copies of the Dirty Dozen & Psalm 151 CD, be sure to complete the bottom portion of the ballot and send payment with your ballot. You do not need to order the CD to participate in the voting process.
  
  

• Individuals who order the CD with their ballot(s) will receive the CD as soon as it becomes available in January 2008.
